We feel that you have made the best choice for your education. Enrolling is easy. Just print each of the forms below. Fill them out completely and get the proper signatures. Then, mail them to us at: 985 Mediterranean Ave., Columbus, OH 43229. We will contact you to tour our school and finalize you enrollment.

This form is the basic application containing all of your personal information. (Required)

Request for Release fo Records

This form allows us to obtain your records from you previous schools. (Required)

Parent-Student Compact

This form outlines the student's and parents responsibilities. (Required)

Technology and Internet Acceptable Use Form

This form outlines how the student uses any of the technology available at our school. (Required)

Emergency Medical Authorization Form

This form allows us to obtain medical treatment in case of an emergency. (Required)

Notice to Parents-Medication Policy

This form allows explains the school's policy regarding students bringing medication to school. (Please review)

Physicians Request for Medication

This form allows us to dispense medication according to the above policy. (Please review)

Inhaler Permission Form

This form allows the student to carry an inhaler according to the above policy. (Please review)
